Information & Referral Services
In many instances, older adults and their families simply do not know where to turn when in need or what programs are available...one answer is this service, commonly referred to as
I & R, which promotes access to other services by providing concrete information to clients about available services or community resources (ie, names, addresses and telephone numbers), as well as providing the client with a referral to an appropriate agency to receive the needed or necessary service. In Region 5 (the counties of Cheyenne, Elbert, Kit Carson and Lincoln), this is provided directly by the Area Agency on Aging though a system of senior services coordinators located in each county.

Outreach Services
Outreach is sometimes known as ‘Client Finding’. This service is usually initiated by a provider to identify new clients and encourage the use of existing services and benefits. This can be accomplished on a one-to-one basis or by the use of flyers, brochures, pamphlets, newsletters, or speaking engagements. In Region 5 (the counties of Cheyenne, Elbert, Kit Carson and Lincoln), this is provided directly by the Area Agency on Aging though a system of senior services coordinators located in each county.

Client Advocacy Service
Client Advocacy is the action or intervention taken on behalf of a person to secure his/her rights or benefits, and includes receiving, investigating and working to resolve disputes or complaints. This does not include services provided by an attorney. In Region 5 (the counties of Cheyenne, Elbert, Kit Carson and Lincoln), this is provided directly by the Area Agency on Aging though a system of senior services coordinators located in each county.

Transportation
Transportation is exactly that, the transporting of a client from one location to another. Anyone, regardless of age, may schedule a ride. Rides are provided for medical and dental appointments, education and employment, congregate meal sites, grocery and essential shopping, legal appointments, and visits to nursing home residents. Many necessary and needed medical services are outside the Region, making travel to the metro area necessary. This service is provided through the Outback Express public transit system. There is a fee for this service, however persons over the age of 60 may purchase a book of tickets valued at $20 for a suggested donation of only $11.

Project SMILE Nutrition Project
SMILE stands for Shared Moments In Living and Eating and this project hopes to do just that, bring seniors together to share moments in their daily lives with others and get a hot, great-tasting, nutritious home-cooked meal to boot!
Congregate Meals...two hot noon meals weekly are provided at a local senior center or nutrition site. In addition to the meal, participants have opportunities to socialize and receive information through educational programs.
Home-Delivered Meals...two hot noon meals weekly are delivered to a client’s home. Frozen meals are also delivered for days when the hot meal is not available.
Nutrition Screening and Education are also provided as part of this service. Screening includes the assessment of an individual’s nutritional health for possible referral to a health care professional. Education is the informal opportunity for an individual to acquire knowledge about nutritional health and its benefits.
These services are provided directly by the Area Agency on Aging, through a system of project kitchens located in each county. Please check the county specific page and county for a location near you.

Services are free to the client, however a suggested donation of $3.00 per meal is encouraged.
***Those over the age of 60 (unless married to someone over 60) are required to pay the guest fee of $5.50 per meal.

In-Home Services include:
Homemaker services include routine house cleaning, laundry, essential shopping, errands, meal preparation, etc. and do not require supervision by a medial person and are services usually performed by domestic workers in order to maintain a healthy and safe environment.
Chore Service is household maintenance such as heavy cleaning (washing walls, windows), scrubbing floor, yard maintenance (mowing lawns, raking leaves), shoveling sidewalks, and replacing light bulbs, etc. These are designed to increase the safety of an individuals residence and do not require a trained homemaker or specialist.
Personal Care services are provided to meet a client’s physical needs and include assisting with basic personal tasks such as bathing, medication, dressing, personal appearance, feeding, etc. These services are provided by personal caregivers under the direction of a medical professional. Also included are Home Health Aide services for persons with acute or chronic illness; and Respite Care, which is short-term home care delivered to an elderly person as a substitute for their regular caregiver. Respite Care is designed to provide relief to relatives and friends who care for persons on a continuous basis.
Eligibility: Persons must be over the age of 60 or the spouse of someone over 60 years of age, however there are other eligibility requirements for these services which are determined by the service provider. Not all providers provide all of the above services, for more information, contact:

National Family Caregiver Support Services
As a result of the reauthorization of the Older Americans Act of 2000, one of the mandated provisions under this act authorized the National Family Caregiver Support Program...established in 2001, this program was designed to respond to the needs of our nation's caregivers.
Eligibility: Services provided under the NFCSP (National Family Caregiver Support Program) are targeted to the family caregivers, grandparents and older individuals who are relative caregivers. Other eligibility requirements may apply, contact us for more information.

Counseling/Support Groups/Training to caregivers to assist in making decisions and solving problems relating to their caregiver role
Respite Care to enable caregivers temporary relief from their care-giving responsibilities
Limited Supplemental Service designed to complement the care provided by caregivers...and may include such services as prescription help, dental and eye care assistance, life-line hookup, minor home modification (wheelchair accessibility)

Legal Services
Legal advice, counseling and representation is provided to eligible clients through agreements with local participating attorneys. This can include assistance in the areas of simple wills, powers of attorney, public benefits and consumer fraud issues. The program will not provide services that include criminal cases, debt problems, consumer collections, foreclosures, or probate/estate issues.

Health Insurance Counseling
Counseling is provided through a contract with Colorado East Community Action Agency. These counselors will provide Medicare and supplemental insurance policy coverage information, including individual policy screening and health insurance needs, to enable clients to make an informed decision about their policy coverage. The counselors cannot and will not recommend a specific policy. This service is typically provided on a one-to-one basis, but can be provided to a group of people. These counselors also provide Medicare Fraud counseling in an effort to stop abuse of the Medicare system. This service is typically provided to a group of people, with individual counseling as needed or necessary.

Ombudsman Services
(pronounced Om - buds - man), Long Term Care Ombudsman are certified to investigate complaints, promote community awareness, and to act as an objective and impartial third party to assist in solving problems involving a variety of issues. The service is comprised of two elements, complaints and education. A complaint is an alleged problem or issue by LTC facility residents, family, friends, and sometimes even the facility itself, in which the Ombudsman takes direct action or suggests action to resolve the complaint. Education is provided in the form of information or training to residents, LTC facility staff, and/or the community regarding resident’s rights or other related issues. Ombudsmen can also provide consultation on long term care options.
